    Millennium Named Participant in ProCare Med Accountable Care Organization (ACO) Physician Group Joins More than 100 New ACOs Just Announced by CMS

    Jan.15, 2013 – Millennium Physician Group has contracted with ProCare Med, LLC, to participate in its ACO to coordinate high quality care to Medicare patients in Millennium’s markets throughout Southwest Florida. ProCare Med is one of just over 100 ACOs recently approved by the Centers for Medicare and Medicaid Services (CMS) to coordinate services to Medicare patients, as announced last week by CMS. CMS states that four million Medicare recipients now have access to care through 250 approved ACOs.

    An ACO consists of a group of doctors, hospitals, and/or other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients. The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors. The primary care physician is the key participant to the patient in ensuring that the ACO’s objectives are met.

    “This is a very exciting announcement for our group,” says Geurt Peet, Chief Executive Officer for Millennium Physician Group. “It’s a goal we’ve been working on for more than a year with our leadership team. Our participation in the ACO will allow us to focus on improved services, improved savings for the patient and a whole new level of care to the patients in the communities we serve.”

    The advantages of ACOs are that they provide improved coordination of care through better outreach and preventive care, resulting in fewer hospitalizations and more comprehensive and effective primary care. At the same time they reduce costs by eliminating duplication and unnecessary treatments and high-cost medical interventions.

    “In the ACO, our doctors will focus with patients on four main areas: improved access to primary care; improved transitions of care between inpatient and outpatient settings; improved population management; and quality medical care,” says Dave Koeninger, Chief Financial Officer for Millennium Physician Group.

    “Millennium has more than 100 providers from Venice to Naples, including hospitalists and primary care physicians,” says Koeninger. “We have the ability to care for our patients in our primary care offices, as well as in the hospitals, ensuring a smooth transition for the patient and the elimination of unnecessary duplication of service. We ensure our patients get the care they need, when they need it, whether they are in our office, imaging centers, labs or local hospitals.”

    ProCare Med is one of 16 new ACOs approved in the state of Florida. Millennium Physician Group will work with ProCare Med to implement the ACO protocols within the group’s practice.
